The San Diego Transformation Center’s Substance Use Disorder Outpatient Program provides continued care and support to help you maintain your progress, prevent relapse, and thrive in long-term recovery. Our OP program ensures you receive consistent, individualized care from the team you know and trust—empowering you to live a healthy, substance-free life.
Who Benefits from OP Care?
Our SUD OP program is ideal for individuals who:
- Have completed detox, PHP, or IOP treatment
- Are stable in early recovery but need ongoing support
- Require medication management or MAT monitoring
- Want to maintain the gains made in treatment
- Need accountability and structure to prevent relapse
- Are managing recovery as part of a long-term wellness plan
Program Structure
- Frequency: 1–2 sessions per week, customized to your needs

• Individual therapy (weekly or bi-weekly)
• Group therapy sessions (optional)
• Psychiatric appointments and medication management (as needed)
• Flexible evening and weekend availability
Your outpatient care is delivered by the same clinical team that supported you through higher levels of care, ensuring seamless continuity and deep therapeutic relationships.

Individual Therapy
- Weekly 50-minute sessions (or as needed)
- Focus on relapse prevention, skill-building, and ongoing recovery goals
Psychiatric Services
- Ongoing medication management (including MAT)
- Regular monitoring and coordination with primary care providers
Optional Group Support
- Weekly process groups
- Relapse prevention and skills reinforcement sessions
- Peer support and alumni connections
Case Management
- Connection to community resources and sober living
- Support with life transitions (employment, housing, legal, family)

Direct Admission Available: You do not need to complete our intensive programs to access OP—new clients are welcome.
