The San Diego Transformation Center’s Substance Use Disorder PHP offers the highest level of outpatient care for individuals struggling with addiction. Our program delivers intensive, structured support in a safe, compassionate environment—empowering you to break free from substance use and rebuild your life, all while returning home each evening.
What Is SUD PHP?
Our Partial Hospitalization Program (PHP) is a comprehensive, evidence-based treatment option for adults who require more support than traditional outpatient care can provide, but do not need 24-hour inpatient hospitalization. PHP is ideal for those who need daily structure, medical oversight, and therapeutic engagement to achieve and maintain sobriety.
Program Structure
- Schedule: 5–6 hours per day, 5-6 days per week
- Consistent Care Team: The same clinicians, therapists, and support staff guide your recovery from intake through aftercare, building trust and accountability.

• Morning check-in and goal setting
• Individual therapy sessions
• Group therapy (process, psychoeducation, and skills)
• Evidence-based modalities (CBT, DBT, relapse prevention, trauma-focused therapy)
• Medication management and psychiatric care

• Afternoon wrap-up and planning
